Each full-length practice test has 85 questions, the minimum length of the real NCLEX-RN, spread across the eight content areas in line with the 2026 test plan percentages, plus 3 clinical judgment case studies of 6 questions each, as on every real exam, with the official 5-hour limit. It is a fixed, linear test with single-answer questions: we do not reproduce the computerized adaptive testing, the passing standard, the 15 unscored pretest items or the NCLEX item formats with several answers and partial credit (such as select-all-that-apply). Our case studies follow the six steps of NCSBN's Clinical Judgment Measurement Model, but each question has one correct answer. A practice score is not a prediction of a pass or fail.

Sample question from practice test 1

At the end of a shift, an RN gives full responsibility for a client's care to the oncoming RN. According to the national delegation guidelines, what is this transfer called?

  1. A delegation
  2. A referral
  3. A supervision agreement
  4. A handoff

Answer D. The national guidelines state that transferring responsibility for a client between licensed providers, such as RN to RN, is a handoff and is not covered by the delegation guidelines. Delegation means giving a specific nursing activity to a delegatee while the delegating nurse keeps accountability for the client.

An AP who was delegated to collect a urine specimen tells the nurse she asked another AP to do it because she was busy. How should the nurse respond?

  1. Thank the AP for finding help so quickly, since the specimen still gets collected on time and teamwork between assistive personnel is encouraged
  2. Tell the AP this is acceptable as long as both assistive personnel work on the same unit
  3. Explain that a delegated task cannot be passed on, and that the AP should tell the nurse when unable to do it
  4. Document that the second AP is now accountable for the client for the rest of the shift

Answer C. Only the licensed nurse can decide whether to delegate an activity to another person; a delegatee who cannot finish must inform the nurse so the nurse can reassess. Accountability for the client stays with the licensed nurse.
